Social egg freezing: for better, not for worse.

چکیده
The possibility for healthy women to cryopreserve their oocytes in order to counter future infertility has gained momentum in recent years. However, women tend to cryopreserve oocytes at an age that is suboptimal from a clinical point of view--in their late thirties--when both oocyte quantity and quality have already considerably diminished and success rates for eventually establishing a pregnancy are thus limited. This also gives rise to ethical concerns, as the procedure is seen as giving false hope to (reproductively speaking) older women. This study evaluates which measures can be taken to turn social freezing into a procedure that is both clinically and ethically better than the current practice. The main objective of these measures is to convince those women who are most likely to (want to) reproduce at an above-average age to cryopreserve their oocytes at a time when this intervention is still likely to lead to a live birth and to discourage fertility clinics from specifically targeting women who have already surpassed the age at which good results can be expected. The possibility for healthy women to cryopreserve their oocytes in order to counter future infertility has gained momentum in recent years. However, women tend to cryopreserve oocytes at a time that is suboptimal from a clinical point of view - in their late thirties - when both oocyte quantity and quality have already considerably diminished and success rates for eventually establishing a pregnancy are thus limited. This also gives rise to ethical concerns, as the procedure is seen as giving false hope to (reproductively speaking) older women. We evaluate which measures can be taken to turn social freezing into a procedure that is both clinically and ethically better than the current practice and discern three different steps: creating public awareness; offering individualized, age-specific information and counselling; and offering predictive tests such as anti-Müllerian hormone measurements or antral follicle count. The main objective of these measures is to convince those women who are most likely to benefit from social freezing to present themselves before age 35 and to discourage fertility clinics from specifically targeting women who have already surpassed the age at which good results can be expected.

I-23: Freezing Oocytes: New or Old Ethical Issues?
Background: This paper considers whether the development of oocyte freezing raises any new or distinctive ethical issues. It will examine the ethical issues raised by this technique: the ’social’ use of egg freezing and how that might be defined; the use of egg freezing for cancer patients; and limitations and benefits of the technique. Materials and Methods: A consideration of the literature a...
متن کاملEffect of egg yolk, cryoprotectant, and various sugars on semen cryopreservation in endangered Cuvier's gazelle (Gazella cuvieri).
Cryopreservation of spermatozoa from endangered species is a valuable tool for genetic management. Previous studies showed the feasibility of cryopreservation of spermatozoa from various endangered gazelles but have also revealed difficulties with available protocols for semen freezing in Cuvier's gazelle (Gazella cuvieri). Experiments were carried out to investigate the effect of (a) 5% or 20%...
متن کاملEgg freezing, stratified reproduction and the logic of not
This commentary examines social and political implications of social egg freezing in a market that is stratified, globalized, and part of a larger bioeconomy. John Robertson's article and public discourse prompted by Facebook and Apple's 'corporate egg freezing' benefits provide touchstones for interrogating social and industry practices that embrace making reproductive capacity marketable. Sup...
متن کاملEgg freezing and egg banking: empowerment and alienation in assisted reproduction
With the development of rapid freezing of human oocytes, many programs have reported IVF success rates comparable to those achieved with fresh eggs and thawed frozen embryos. Egg freezing is now gaining professional and regulatory acceptance as a safe and effective technique for women who wish to avoid discarding excess embryos, who face fertility-threatening medical treatments, or who want to ...
متن کاملO-20: Slow-Freezing-Induced Changes of Birefringent Structures in Human Oocytes Are Related to Responsiveness to Ovulation Induction
Background: The slow-freezing method is one of the most popular techniques to freeze human oocytes and it is also used in routine in vitro-fertilization (IVF) programs, particularly in those countries where embryo freezing is not allowed. Slow-freezing damages some of the structures of the egg, among which the meiotic spindle (MS) and the zona pellucida (ZP). Polarized Light Microscopy (PLM) pe...
